Allergy Clinic Patient Information Sheet | Sheree

Patient Name: Sheree
Age: 13 years old
Sex: Male

Reason for Visit to the Clinic: Sheree’s mother brought her to the clinic two days after taking her to the emergency room. Two days ago, the family had gone to lunch with friends and then to the park. At the park, the kids started running and playing. Sheree found her dad and showed him that she had developed hives—small, red, itchy bumps on her skin. Her nose was running and her eyes were watering. She also started complaining about having trouble breathing.

Other information: Sheree’s parents told the clinic staff that her symptoms started about 2 hours after eating lunch. They also said that Sheree had never had any problems like this before.
