Allergy Clinic Patient Information Sheet | Zach

Reason for Visit to the Clinic: Zach’s mother took him to the emergency room two days ago. While at his neighbor Jason’s house, Zach developed hives—small, red, itchy bumps on his skin. Jason called Zach’s mom. His throat also tightened up, making it difficult to swallow. Zach and his mom got really scared when he started having trouble breathing.

Other information: At the hospital, Zach told the doctors that the problems started after he ate some snacks—crackers and cookies. Zach and his mom both told the nurse that he had had a reaction to peanuts before, but Zach didn’t think that the snacks had any peanuts in them.
